The following applications may either be filled out on the computer and then printed or printed and then typed or hand written.  At this time, they may not be submitted on line. Please note that only the information showing in the text boxes will appear when the application in printed. Upon completion, please mail the application to:

• Application for Classified (degree-seeking) Student
  Please refer to the individual department pages to determine admission requirements for a particular department.

• Recommendation Forms
  Please refer to the individual department pages to determine if letters of recommendation are required.

• School of Public Health Supplemental Application
  Must be completed by all applicants to the School of Public Health degree programs (Master of Community Health, Master of Health Administration and Doctor of Philosophy in Health Services Research).

• International Student Application
  This preliminary application MUST be filled out by all international students and be accepted by The Graduate School before international students can submit an Application for Classified (degree-seeking) Student.
